IMPORTANT! If you are on this list, remember that you and your parent are to be at the OMS softbal field at 9:00 a.m. Saturday. If you can, bring tillers, shovels, or whatever you have to get the grall of the infield and whatever you can to clean the asphalt around the field.

Parents Needed! Attention softball parents. If you would like to help work in the concession stands for home games or donate items to be sold, we need you . Please contact Coach Crawford if you are interested.

Sports Physicals: No one can participate without proof that you have had a physical sometime after April 15th of ths year!

Consent and Insurance Forms: Parents are to read consent and Insurance forms carefully. Each one should be signed by both the parent or legal guardian and the athlete. If the athlete is not covered by the family insurance, purchase of school insurance is required.

Personal Information and Previous Participation: Please fill out accurately. This will help us organize.

Emergency Sheets: Please fill out all blanks carefully and completely. Theis will be taken up from each athlete and carried by coaches to each practice and game in case of emergency.

Eligibility: Athletes are not considered eligible to participate until all paperwork is turned in and completed to the satisfaction of the OMS coaching staff. NO EXCEPTIONS.

Tryout and Practice Location: All conditionings and practices will take place at OMS softball field.

Transportation: Parents are expected to make arrangements for transportation to and from conditionings and practices and are expected to exhibit courtesy to coaching staff by being on time for conditionings and practices and picking athletes up after conditionings and practices.

Equipment Needed: All athletes must bring their own glove and softball shoes.
